How to Get Started at our Office:

  • Call our South office at (702) 434-6920 or Northwest office at (702) 998-5200 to schedule an exam with the provider.

  • Bring to first appointment:

    • Any pertinent imaging studies including x-rays, MRIs, CTs, and ultrasounds

    • ​Outside medical records and hospital discharge papers

    • Print and complete our patient packet (PDF, right) and bring it to your first visit.

    • Identification (driver's license or other government-issued ID)

    • If you are not a biological parent, please bring papers of custody/guardianship for minor child

    • Insurance Cards

    • Insurance forms that may be required by your insurance (see below)

  • Finally: Show up at your scheduled time

  • Regarding face coverings, we follow current guidance from the CDC, FDA and Nevada OSHA.

  • We look forward to your visit!  Our priority is to help you address your condition as safely as possible.

  • ​What Can You Expect When You Walk into the Office for the First Time?

Our front desk staff will greet you and take your paperwork. They'll also ask you for your Identification card and your insurance card so that they can make copies for the office's records.

  • ​What can you anticipate during your appointment?

During your visit, the provider or the medical assistants will ask you questions about your medical history and your orthopedic concerns. This will be followed by a focused orthopedic evaluation that may include obtaining x-rays, that will help us to identify a diagnosis and plan of care for your child.

  • ​Do you have to get X-Rays?

X-rays will only be taken if the provider feels it is necessary to rule in or out specific conditions. If you have previously completed x-rays with the urgent care, hospital, or previous doctors, please bring these to your appointment.
